The purpose of the Asthma Disease Management Program is to improve the health status and quality of life while decreasing unnecessary hospitalizations and emergency room visits by members with asthma through improved compliance of both members and practitioners with Pediatric Associates Health Plan Clinical Practice Guidelines, which are based the National Institutes of Health (NIH) standards of care.

Child Health Check Up Management Program Description
The CHCUP Program is designed to provide a comprehensive preventive healthcare package to members from birth to age 21 and allow for the early diagnosis and treatment of medical conditions which, if undetected, could result in serious medical conditions and/or costly medical care. Interventions are focused on the member, the practitioner and the community. The CHCUP Program includes the maintenance of a coordinated system to follow the Member through the entire range of screening and treatment, as well as supplying CHCUP training to medical care providers.
